COVID-19 Quarantine Guidelines

2024-2025 Moderna (Spikevax) COVID-19 vaccine

We have the updated Moderna (Spikevax) COVID-19 vaccine for ages 18 years and older. We do not have the updated Pfizer (Comirnaty) or Novavax COVID-19 vaccines. No appointment necessary. Walk in between 11 AM and 6 PM Monday through Friday, between 11 AM and 4 PM on Saturday, or between 11 AM and 1:30 PM on Sunday.

STEP 1

Click the "Vaccine Consent Form" link below to complete the necessary paperwork to receive the COVID-19 vaccine. Print out the page, fill it out, and bring it with you.
STEP 2

You may walk into pharmacy any day to receive the vaccine. Walk in between 11 AM and 6 PM Monday through Friday, 11AM and 4 PM on Saturday, or 11 AM through 1:30 PM on Sunday; no appointment is necessary. Bring a copy of your completed paperwork with you.
